Part D Late Enrollment Penalty If you go 63 or more consecutive days without creditable prescription drug coverage after becoming eligible for Medicare, you face a permanent Part D penalty. The penalty is 1% of the national base beneficiary premium for every month without coverage.

Medigap Penalty (Loss of Guaranteed Issue Rights) This is not a financial penalty per se, but it is equally costly. If you miss your 6-month Medigap Open Enrollment Period (which starts when you enroll in Part B at 65), you lose the right to enroll in any Medigap plan without medical underwriting. Insurers can deny you coverage or charge higher premiums based on your health.
