The most important Medicare decision most beneficiaries make is choosing between Medicare Advantage (Part C) and Original Medicare with a Medigap supplement. Both are legitimate, widely used approaches -- but they work very differently. Here is an honest, comprehensive comparison.

How Each Approach Works

Original Medicare + Medigap:

  • Medicare pays 80% of covered services
  • Medigap pays most or all of the remaining 20% plus deductibles
  • You use any doctor or hospital that accepts Medicare -- nationwide, no network restrictions
  • Separate Part D plan for prescription drugs

Medicare Advantage:

  • A private insurance plan that replaces Original Medicare
  • Covers all Part A and Part B services (and usually Part D)
  • Restricts care to a network of providers
  • Lower or $0 premiums, but cost-sharing at point of service
  • Extra benefits (dental, vision, hearing, OTC)

The tradeoff: Medigap costs more monthly but provides near-unlimited protection. Medicare Advantage costs less monthly but exposes you to potentially thousands in out-of-pocket costs if you have a serious illness or hospitalization.

Provider Access

Medigap: See any doctor or hospital in the country that accepts Medicare -- no referrals, no network restrictions. Particularly valuable for:

  • Snowbirds who split time between states
  • People who travel frequently
  • People with established relationships with specific specialists
  • People who want access to major academic medical centers

Medicare Advantage: Restricted to the plan's network. HMOs require referrals; PPOs allow out-of-network care at higher cost. Network quality varies significantly by plan and location.

Predictability of Costs

Medigap: Highly predictable. Once you've paid the Part B deductible ($283 in 2026), Plan G covers virtually everything else. No surprise bills.

Medicare Advantage: Less predictable. Copays and coinsurance accumulate with each service. A hospitalization, surgery, or serious illness can generate thousands in cost-sharing -- up to the out-of-pocket maximum.

Extra Benefits

Medigap: No extra benefits beyond Medicare coverage. No dental, vision, or hearing.

Medicare Advantage: Often includes dental, vision, hearing, OTC allowance, fitness membership, and other extras. Value varies significantly by plan.

Switching Between the Two

From Medigap to MA: You can switch to Medicare Advantage during Annual Enrollment Period (Oct 15-Dec 7). Your Medigap policy doesn't automatically cancel -- you must contact your insurer.

From MA to Medigap: You can disenroll from MA during AEP and return to Original Medicare -- but getting Medigap may require medical underwriting (health questions). Outside of your initial Medigap enrollment period, insurers can deny coverage or charge higher premiums based on health history.

This asymmetry is critical: It is much easier to switch from Medigap to MA than from MA to Medigap. If you start with MA and later want Medigap, you may not be able to get it at standard rates.

  • People with chronic conditions requiring frequent specialist care
  • People who value predictable costs over lower premiums
  • Snowbirds and frequent travelers
  • People who want access to any Medicare provider nationwide
