International travel is one of the great joys of retirement -- but many seniors don't realize that their Medicare coverage essentially stops at the US border. Understanding this gap and planning accordingly can prevent a financial catastrophe if you need medical care overseas.
What Original Medicare Covers Abroad
Original Medicare (Parts A and B) covers healthcare services in the United States, its territories (Puerto Rico, US Virgin Islands, Guam, American Samoa, Northern Mariana Islands), and on US-flagged ships within 6 hours of a US port.
Outside these areas, Original Medicare pays nothing -- with three narrow exceptions:

Canadian border emergency: If you're traveling through Canada between Alaska and the contiguous US by the most direct route, and a medical emergency occurs, Medicare may cover care at a Canadian hospital if it's closer than the nearest US hospital.

Mexican border emergency: Similar rules apply if you're in the US but closer to a Mexican hospital than a US hospital.

US ship within 6 hours of port: Medicare covers care on a US ship if the ship is within 6 hours of a US port when the emergency occurs.
These exceptions are narrow and rarely applicable to typical international travel.
Medigap Plans F, G, and N: Foreign Travel Emergency Coverage
Several Medigap plans include a foreign travel emergency benefit -- one of the most valuable features for traveling seniors.
Medigap plans with foreign travel coverage: C, D, F, G, M, and N
What it covers: 80% of medically necessary emergency care outside the US, after a $250 deductible, up to a lifetime maximum of $50,000.
What it does NOT cover: Non-emergency care, routine care, or care that could have waited until you returned to the US.
The $50,000 lifetime maximum is a significant limitation -- a serious illness or injury abroad can easily exceed this amount. But it provides meaningful protection for most travel emergencies.
Medicare Advantage and International Travel
Medicare Advantage plans vary in their international coverage. Most MA plans cover emergency care abroad, but coverage limits and cost-sharing differ by plan. Check your plan's Evidence of Coverage for specific international coverage details.
Travel Insurance: The Essential Supplement
For serious international travelers, dedicated travel insurance is essential. Travel medical insurance provides:
- Higher coverage limits ($100,000-$1,000,000+)
- Medical evacuation coverage (can cost $50,000-$200,000 without insurance)
- Trip cancellation and interruption coverage
- 24/7 assistance services
Medical evacuation insurance is particularly important for seniors. If you have a serious medical emergency in a country with limited medical facilities, evacuation to a facility capable of treating you can cost more than $100,000.

Tips for International Travel with Medicare

Purchase travel medical insurance for every international trip -- don't rely on Medigap's $50,000 limit for extended travel.

Carry documentation of your conditions and medications -- a list of diagnoses, current medications (generic names), allergies, and your doctor's contact information.

Research medical facilities at your destination before you go. Know where the best hospitals are and how to access emergency care.

Consider a medical alert bracelet if you have serious conditions.

Check your Medigap plan -- if you have Plan G or F, you have the foreign travel emergency benefit. Know the $250 deductible and $50,000 lifetime limit.

Cruise ships: Most cruise lines have medical facilities on board, but care is expensive. Travel insurance with medical coverage is strongly recommended for cruises.
