Obesity affects more than 40% of Americans over 60, and it is a major driver of heart disease, diabetes, joint problems, and other conditions that Medicare covers extensively. Yet many seniors do not know that Medicare itself covers several obesity treatments.

Here is a complete breakdown of what Medicare covers for weight loss and obesity in 2026.

Medicare Part B: Obesity Counseling

Original Medicare (Part B) covers Intensive Behavioral Therapy (IBT) for Obesity at no cost to you -- no copay, no deductible -- when provided by a primary care doctor in a primary care setting.

Who qualifies: Beneficiaries with a Body Mass Index (BMI) of 30 or higher.

What is covered:

  • One face-to-face visit per week for the first month
  • One visit every other week for months 2-6
  • One visit per month for months 7-12 (if you meet the 3kg weight loss goal at 6 months)

Total: Up to 22 visits in the first year, all at $0 cost to you.

This is a genuinely valuable benefit that most Medicare beneficiaries never use. If your BMI is 30 or above, ask your primary care doctor about IBT at your next visit.

Medicare Part B: Bariatric Surgery

Medicare covers bariatric surgery (weight loss surgery) when it is deemed medically necessary. Covered procedures include:

  • Roux-en-Y gastric bypass
  • Laparoscopic adjustable gastric banding
  • Sleeve gastrectomy

Requirements for coverage:

  • BMI of 35 or higher with at least one obesity-related condition (type 2 diabetes, hypertension, sleep apnea, etc.)
  • Documentation that other weight loss methods have failed
  • Surgery performed at a Medicare-certified bariatric surgery center of excellence

Under Original Medicare, you pay the Part B deductible ($283 in 2026) plus 20% coinsurance for the surgery. With Medigap Plan G, your out-of-pocket cost is just the $283 deductible.

GLP-1 Medications: The 2026 Update

GLP-1 receptor agonists -- including semaglutide (Ozempic, Wegovy) and tirzepatide (Mounjaro, Zepbound) -- have transformed obesity treatment. These medications can produce 15-20% body weight reduction in clinical trials.

Medicare Part D coverage for GLP-1s:
