Medicare Dental, Vision, and Hearing Coverage: What Is and Is Not Covered

Dental disease, vision problems, and hearing loss are among the most common health issues facing seniors -- yet Original Medicare covers almost none of these services. This coverage gap affects millions of beneficiaries and can lead to significant out-of-pocket costs. Here is what you need to know.

Original Medicare: The Coverage Gap

Dental: Original Medicare (Parts A and B) does not cover routine dental care -- cleanings, fillings, extractions, dentures, or dental implants. Medicare Part A covers dental services only when they are an integral part of a covered medical procedure (e.g., jaw reconstruction after an accident, or dental exam required before a heart valve replacement).

Vision: Original Medicare does not cover routine eye exams for glasses or contact lenses, eyeglasses, or contact lenses. Medicare does cover medically necessary eye care -- treatment for glaucoma, macular degeneration, diabetic retinopathy, and cataract surgery (including one pair of standard eyeglasses after cataract surgery).

Hearing: Original Medicare does not cover routine hearing exams or hearing aids. Medicare does cover diagnostic hearing exams ordered by a physician to determine a medical cause of hearing loss.

Medicare Advantage: Filling the Gap

Many Medicare Advantage plans include dental, vision, and hearing benefits as supplemental coverage -- one of the most significant advantages of MA over Original Medicare + Medigap.

Dental benefits (typical MA coverage):

  • Preventive care (cleanings, X-rays, exams): Often covered at 100%
  • Basic restorative (fillings, simple extractions): 50-80% coverage
  • Major restorative (crowns, root canals, dentures): 50% coverage, often with annual limits
  • Annual benefit maximum: $1,000-$3,000 (some plans offer higher limits)
  • Orthodontics: Rarely covered for adults

Vision benefits (typical MA coverage):

  • Annual eye exam: Covered
  • Eyeglasses or contact lens allowance: $100-$300/year
  • Some plans cover LASIK at a discount

Hearing benefits (typical MA coverage):

  • Annual hearing exam: Covered
  • Hearing aid allowance: $500-$2,500 per ear (every 1-3 years)
  • Some plans partner with hearing aid networks for additional discounts

Important: MA dental, vision, and hearing benefits vary enormously between plans. Compare benefits carefully -- a plan with rich dental coverage may have poor hearing benefits, and vice versa.

Standalone Dental and Vision Insurance

If you have Original Medicare with Medigap (no MA plan), you can purchase standalone dental and vision insurance:

Dental insurance: Plans typically cost $20-$50/month. Look for plans with no waiting period for preventive care and reasonable annual maximums.

Vision insurance: Plans typically cost $10-$20/month. Most cover one annual exam and a glasses/contacts allowance.

Dental discount plans: Not insurance -- membership programs that provide discounted rates at participating dentists. Can be cost-effective for people who need significant dental work.

Medicaid Dental Coverage

Florida Medicaid covers limited dental services for qualifying low-income beneficiaries -- primarily emergency extractions and dentures. Coverage is more limited than most private dental plans.

Community Dental Resources

For seniors who cannot afford dental care:

  • Dental schools: Provide care at significantly reduced rates under faculty supervision
  • Community health centers: Federally Qualified Health Centers (FQHCs) provide dental care on a sliding fee scale
  • Florida Department of Health dental clinics: Some county health departments offer dental services
