Step 1: Understand What Medicare Covers
Medicare has four parts:
- Part A -- Hospital insurance. Covers inpatient hospital stays, skilled nursing facility care, hospice, and some home health care. Most people pay $0 premium for Part A if they or their spouse worked at least 10 years and paid Medicare taxes.
- Part B -- Medical insurance. Covers doctor visits, outpatient care, preventive services, and durable medical equipment. The standard 2026 premium is $202.90/month.
- Part C (Medicare Advantage) -- An alternative to Original Medicare offered by private insurers. Bundles Parts A, B, and usually D into one plan.
- Part D -- Prescription drug coverage. Available as a standalone plan (with Original Medicare) or bundled into a Medicare Advantage plan.
Step 2: Know Your Enrollment Periods
Initial Enrollment Period (IEP)
Your first opportunity to enroll in Medicare. It runs for 7 months -- 3 months before your 65th birthday month, your birthday month, and 3 months after.
Enroll as early as possible within this window. If you wait until the months after your birthday, your coverage start date is delayed.
General Enrollment Period (GEP)
If you missed your IEP and do not qualify for a Special Enrollment Period, you can enroll during the General Enrollment Period: January 1 - March 31 each year. Coverage begins July 1. Late enrollment penalties may apply.
Special Enrollment Period (SEP)
You qualify for an SEP if you experience certain life events, including:
- Losing employer-sponsored health coverage
- Moving to a new service area
- Your Medicare Advantage plan exiting your county
- Qualifying for Extra Help (Low Income Subsidy)
SEPs typically last 2-3 months from the triggering event. Do not miss them.
Annual Enrollment Period (AEP)
October 15 - December 7 each year. During AEP, any Medicare beneficiary can:
- Switch from Original Medicare to Medicare Advantage (or vice versa)
- Switch between Medicare Advantage plans
- Join, switch, or drop a Part D plan
Changes made during AEP take effect January 1 of the following year.
Medicare Advantage Open Enrollment Period (OEP)

If you choose Original Medicare with a Medigap plan, you must enroll in a standalone Part D plan for prescription drug coverage. Failing to do so -- and going 63 or more days without creditable drug coverage -- results in a permanent late enrollment penalty.
In 2026, the Part D out-of-pocket cap is $2,100 -- the most significant drug coverage improvement in the program's history.
