Medicare Coverage for Stroke Recovery: Rehabilitation, Therapy, and Long-Term Care
Stroke is a leading cause of long-term disability in the United States. Here is what Medicare covers for stroke recovery -- from acute hospitalization through rehabilitation and ongoing therapy.
Medicare Coverage for Stroke Recovery: Rehabilitation, Therapy, and Long-Term Care
Stroke is the fifth leading cause of death and the leading cause of long-term disability in the United States. Each year, approximately 800,000 Americans have a stroke -- and the majority are Medicare beneficiaries. Medicare covers a comprehensive continuum of stroke care, from emergency treatment through long-term rehabilitation.
Acute Stroke Hospitalization (Part A)
When a stroke occurs, immediate hospitalization is essential -- "time is brain." Medicare Part A covers inpatient hospitalization for acute stroke treatment:
- Emergency evaluation and stabilization
- Brain imaging (CT, MRI)
- IV tPA (clot-busting medication) for ischemic stroke
- Mechanical thrombectomy (catheter-based clot removal)
- Neurology and neurosurgery consultations
- ICU monitoring
- Medications during hospitalization
- Initial rehabilitation assessment
Cost: Part A deductible per benefit period ($1,556 in 2022). Medigap Plan G covers this deductible.
Inpatient Rehabilitation Facility (IRF) (Part A)
After acute stroke hospitalization, many patients transfer to an Inpatient Rehabilitation Facility (IRF) -- a specialized facility providing intensive rehabilitation.
IRF criteria: To qualify for IRF admission, patients must be able to tolerate and benefit from at least 3 hours of therapy per day, 5 days per week.
What IRF provides:
- Physical therapy (mobility, strength, balance)
- Occupational therapy (daily living activities, arm/hand function)
- Speech-language pathology (communication, swallowing)
- Recreational therapy
- Neuropsychology
- 24-hour nursing care
Medicare coverage: Covered under Part A. The IRF stay is counted separately from the acute hospital stay for benefit period purposes.
Skilled Nursing Facility (SNF) Rehabilitation (Part A)
For patients who cannot tolerate the intensity of IRF, a skilled nursing facility provides less intensive rehabilitation -- typically 1-2 hours of therapy per day.
Coverage: Up to 100 days per benefit period after a qualifying 3-day inpatient hospital stay. Days 1-20 at no cost; days 21-100 with daily coinsurance ($194.50/day in 2022). Medigap Plan G covers the coinsurance.
Home Health Rehabilitation (Part B)
After discharge from a facility, homebound stroke survivors can receive rehabilitation at home:
- Physical therapy
- Occupational therapy
- Speech-language pathology
- Skilled nursing visits
- Home health aide services
Coverage: No cost-sharing for home health visits (covered at 100% under Part B).
Outpatient Therapy (Part B)
Once a stroke survivor is no longer homebound, outpatient therapy continues recovery:
- Physical therapy at an outpatient clinic or hospital
- Occupational therapy
- Speech-language pathology
Coverage: 20% coinsurance after the Part B deductible. Medigap Plan G covers this 20%.
Therapy cap: Medicare eliminated the therapy cap in 2018 -- there is no longer a dollar limit on outpatient therapy. Coverage continues as long as therapy is medically necessary and the patient is making progress.
Long-Term Stroke Care
For stroke survivors with permanent disability, Medicare covers ongoing management:
- Regular physician visits
- Medications (Part D): Blood thinners, blood pressure medications, statins, antidepressants
- Durable medical equipment: Wheelchair, walker, AFO brace, communication devices
- Chronic Care Management for coordinated care
- Mental health services (depression is common after stroke)
Stroke Prevention
Medicare covers services to prevent recurrent stroke:
- Blood pressure monitoring and management
- Atrial fibrillation screening and treatment
- Carotid artery imaging when indicated
- Anticoagulation management
- Cardiovascular risk factor management
