Medicare vs. Medicare Advantage in Florida 2026: Which Saves Retirees More?

"Is Medicare Advantage worth it?" is the question I get more than any other.

The honest answer: it depends entirely on your health, your doctors, and how you use healthcare. There's no universal right answer -- but there is a right answer for your specific situation.

Here's the complete comparison for Florida retirees in 2026.

The Two Paths: How They Work

Path 1: Original Medicare (Parts A + B) + Medigap + Part D

  • Part A covers hospital stays, skilled nursing, hospice
  • Part B covers doctor visits, outpatient care, preventive services
  • Medigap (Medicare Supplement) covers most of what Parts A and B don't pay
  • Part D covers prescription drugs

How it works: Medicare pays first (80% of Part B costs after deductible), Medigap pays second (the remaining 20%), and Part D covers drugs. With Plan G, you pay essentially nothing after the Part B deductible ($283 in 2026).

Provider access: Any Medicare-accepting provider in the country. No networks, no referrals, no prior authorization for most services.

Path 2: Medicare Advantage (Part C)

A Medicare Advantage plan replaces Original Medicare. The plan is run by a private insurer (Humana, Aetna, Florida Blue, etc.) that contracts with Medicare to provide your Part A and B benefits -- usually with extras like dental, vision, and hearing.

How it works: You pay the plan's copays and coinsurance when you use care. Most Florida MA plans have $0 premiums in 2026.

Provider access: In-network providers only (HMO) or any provider with higher cost-sharing for out-of-network (PPO). Prior authorization required for many services.

Cost Comparison: Florida 2026

Scenario 1: Healthy Senior, Minimal Healthcare Use

Original Medicare + Plan G + Part D:

  • Part B premium: $202.90/month
  • Plan G premium: $140/month (age 65, female, non-smoker, Volusia County)
  • Part D premium: $30/month
  • Part B deductible: $283/year
  • Total annual cost: ~$4,277

Medicare Advantage (HMO, $0 premium):

  • Part B premium: $202.90/month
  • MA premium: $0/month
  • Copays for 4 doctor visits: $0-$80
  • Total annual cost: ~$2,300

Winner for healthy seniors: Medicare Advantage -- saves $1,500-$2,100/year.

Scenario 2: Active Healthcare User (Multiple Specialists, Chronic Condition)

Original Medicare + Plan G + Part D:

  • Part B premium: $202.90/month
  • Plan G premium: $140/month
  • Part D premium: $30/month
  • Part B deductible: $283/year
  • Specialist visits: $0 (covered by Plan G after deductible)
  • Total annual cost: ~$4,277 (predictable, no surprises)

Medicare Advantage (HMO, $0 premium):

  • Part B premium: $202.90/month
  • MA premium: $0/month
  • 20 specialist visits at $40 copay: $800
  • 2 diagnostic tests (MRI, CT): $200-$400
  • Prior authorization delays: potential
  • Total annual cost: ~$3,500-$4,000

Winner for active healthcare users: Medigap -- comparable cost with zero surprises and no prior authorization.

Scenario 3: High-Cost Healthcare User (Surgery, Hospitalization)

Original Medicare + Plan G + Part D:

  • Total annual cost: ~$4,277 (same as above -- Plan G covers everything after deductible)

Medicare Advantage (HMO, $0 premium, $3,400 OOP max):

  • Part B premium: $202.90/month
  • Hospital stay: $500-$1,500 in copays
  • Surgery: $500-$2,100 in copays
  • Specialist visits: $400-$800
  • Total annual cost: up to $5,620 (Part B premium + $3,400 OOP max)

Winner for high-cost users: Medigap -- Plan G's predictable costs beat MA's OOP maximum.

The Florida-Specific Factors

South Florida (Miami-Dade, Broward, Palm Beach)

South Florida has the most Medicare Advantage competition in the state -- and some of the most aggressive plan benefits. $0-premium plans with $2,100 OOP caps and comprehensive dental are common.

However, South Florida also has some of the highest healthcare utilization rates in the country. If you use healthcare frequently, the math often favors Medigap despite the higher premium.

Central Florida (Orlando, Tampa, Clearwater)

Central Florida has excellent MA options from Humana, Aetna, and Florida Blue. The AdventHealth and BayCare networks are well-covered by most major carriers.

Northeast Florida (Jacksonville, Daytona Beach, Palm Coast)

The Volusia/Flagler market has strong options from Aetna, Humana, and UnitedHealthcare. Halifax Health network participation varies by carrier -- always verify before enrolling.

Rural Florida (Panhandle, North Central FL)

Rural areas have fewer MA plan options and sometimes weaker networks. Medigap is often the better choice in rural counties where MA networks are thin.

The 5 Questions That Determine Your Best Choice

1. Do you have established relationships with multiple specialists? If yes, Medigap is safer -- no network restrictions, no referrals, no prior authorization.

2. Do you travel frequently or split time between states? If yes, Medigap or a PPO MA plan. HMO plans only cover emergency care outside their service area.

3. Are you healthy and rarely use healthcare? If yes, Medicare Advantage likely saves you money -- especially with a $0 premium and low OOP cap.

4. Do you take expensive specialty medications? The $2,100 Part D OOP cap applies to both MA and standalone Part D plans. This levels the playing field for drug costs.

5. Do you value predictability over savings? Medigap gives you a fixed monthly cost with no surprises. MA plans have variable costs depending on how much care you use.

The Bottom Line

Neither path is universally better. The right choice depends on your health, your doctors, your prescriptions, and your risk tolerance.

What I do for every client: run the actual numbers for their specific situation -- their doctors, their medications, their ZIP code -- and show them exactly what each option costs. That's the only way to make an informed decision.
