Medicare Part B and Part D premiums are higher for beneficiaries whose income exceeds certain thresholds. This surcharge is called IRMAA (Income-Related Monthly Adjustment Amount). Social Security uses your income from two years prior to determine whether IRMAA applies.
IRMAA is tiered — the higher your income, the higher the surcharge. For 2025, IRMAA applies to individuals with modified adjusted gross income (MAGI) above $106,000 and married couples filing jointly above $212,000. These thresholds are adjusted annually for inflation.
If your income has decreased significantly since the year used to calculate your IRMAA (due to retirement, divorce, death of a spouse, or other life-changing events), you can appeal to Social Security using Form SSA-44. A successful appeal can reduce or eliminate your IRMAA surcharge.
IRMAA is separate from your Medicare Advantage or Supplement premium — it is added to your Part B and Part D base premiums. An independent broker can help you understand your total Medicare cost picture.
