No out-of-pocket maximum in Original Medicare. Without a Medigap (Medicare Supplement) plan, the 20% coinsurance has no cap. A $100,000 surgery leaves you with $20,000 in out-of-pocket costs. This is why most people add a Medigap plan or choose Medicare Advantage.
If your income exceeds certain thresholds, you pay an Income-Related Monthly Adjustment Amount (IRMAA) on top of your standard Part B and Part D premiums. IRMAA is based on your 2024 tax return (two years prior).

Original Medicare has no out-of-pocket maximum — the 20% Part B coinsurance is unlimited. Medicare Advantage plans must cap out-of-pocket costs at $9,350 for in-network services in 2026. Medigap plans can eliminate most or all out-of-pocket costs.
The penalty is 10% of the standard premium for each full 12-month period you were eligible but did not enroll. It is permanent. At $202.90/month standard, a 2-year delay adds $37/month — $444/year — for life.
