The Real Cost of Medicare in Florida: Premiums, Deductibles & Copays
Medicare is not free. Here is a complete breakdown of what Florida seniors actually pay for Medicare in 2026 -- and how to keep those costs as low as possible.

The Real Cost of Medicare in Florida: Premiums, Deductibles & Copays
One of the most common misconceptions about Medicare is that it is free. It is not. While Medicare is far less expensive than most private insurance, there are real costs involved -- and understanding them is essential for retirement planning.
Here is a complete breakdown of what Medicare actually costs in Florida in 2026.
Medicare Part A Costs
Most people get Part A (hospital insurance) for free if they or their spouse worked and paid Medicare taxes for at least 10 years (40 quarters).
If you do not qualify for premium-free Part A, you pay:
- $284/month if you have 30-39 quarters of Medicare-covered employment
- $518/month if you have fewer than 30 quarters
Part A deductibles and cost-sharing (2026):
- Inpatient hospital deductible: $1,736 per benefit period
- Hospital coinsurance: $0 for days 1-60; $419/day for days 61-90; $838/day for days 91-150 (lifetime reserve days)
- Skilled nursing facility: $0 for days 1-20; $209.50/day for days 21-100; all costs after day 100
Medicare Part B Costs
Part B (medical insurance) has a monthly premium for everyone.
Standard Part B premium (2026): $202.90/month
Higher-income beneficiaries pay more through Income-Related Monthly Adjustment Amounts (IRMAA):
| Individual Income | Joint Income | Monthly Premium |
|---|---|---|
| Up to $106,000 | Up to $212,000 | $202.90 |
| $106,001-$133,000 | $212,001-$266,000 | $259.00 |
| $133,001-$167,000 | $266,001-$334,000 | $370.00 |
| $167,001-$200,000 | $334,001-$400,000 | $480.90 |
| Above $200,000 | Above $400,000 | $591.90 |
Part B deductible (2026): $283/year
After the deductible, Original Medicare pays 80% of covered services. You pay the remaining 20% -- with no cap.
The 20% Problem
The 20% coinsurance under Original Medicare is where costs can really add up. There is no out-of-pocket maximum under Original Medicare alone.
Example: A $50,000 surgery under Original Medicare leaves you with a $10,000 bill (20% coinsurance) -- plus the Part B deductible.
This is why most Florida seniors add either a Medicare Supplement (Medigap) plan or a Medicare Advantage plan.
Medicare Supplement (Medigap) Costs in Florida
Medigap plans fill the gaps in Original Medicare. Premiums vary by plan type, age, gender, tobacco use, and zip code.
Estimated monthly premiums for a 65-year-old non-smoker in Florida (2026):
- Plan G: $120-$180/month
- Plan N: $90-$140/month
- High-Deductible Plan G: $40-$70/month
With Plan G, your total annual out-of-pocket exposure is just the Part B deductible ($283/year) -- making your healthcare costs highly predictable.
Medicare Advantage Costs in Florida
Medicare Advantage plans often have $0 or very low monthly premiums, but you pay more when you use care.
Typical Medicare Advantage costs:
- Monthly premium: $0-$50 (many plans are $0)
- Primary care copay: $0-$15
- Specialist copay: $20-$50
- Inpatient hospital: $250-$500/day or a flat copay
- Annual out-of-pocket maximum: $3,000-$8,000
The out-of-pocket maximum is important -- it is the most you will pay in a year. But reaching that maximum can still mean thousands of dollars in costs.
Medicare Part D Costs
Part D (prescription drug) costs vary by plan and by the drugs you take.
- Monthly premium: $10-$100+ depending on plan
- Annual deductible: Up to $590 (some plans have $0 deductibles for certain tiers)
- Annual out-of-pocket cap: $2,100 (2026)
Total Annual Medicare Costs: A Realistic Budget
Here is what a typical Florida senior might budget for Medicare in 2026:
With Original Medicare + Plan G + Part D:
- Part B premium: $2,220/year
- Plan G premium: ~$1,680/year
- Part D premium: ~$360/year
- Part B deductible: $283/year
- Total: ~$4,517/year (with very predictable costs)
With Medicare Advantage ($0 premium) + Part D:
- Part B premium: $2,220/year
- Medicare Advantage premium: $0
- Part D: often included
- Copays and coinsurance: varies by usage
- Total: $2,220/year + copays (lower if healthy, potentially much higher if you need significant care)
