Most people know that Medicare Advantage plans can have $0 premiums. But fewer people know that some plans actually pay you back a portion of your Medicare Part B premium every month.

It sounds too good to be true. It is not. Here is exactly how it works.

What Is the Part B Giveback Benefit?

The Part B giveback benefit -- officially called the "Part B premium reduction" -- is a feature available on some Medicare Advantage plans. Instead of charging you a premium, the plan credits a portion of your Part B premium back to your Social Security check (or directly to you if you pay Part B directly).

In 2026, the standard Medicare Part B premium is $202.90 per month. Some giveback plans reduce that by anywhere from $10 to the full $185.00.

If you receive a $100/month giveback, your effective Part B premium drops from $185.00 to $102.90. That is $1,200 back in your pocket every year.

How Does the Giveback Work Mechanically?

When you enroll in a Medicare Advantage plan with a Part B giveback benefit:

  1. You continue paying your Part B premium as normal (usually deducted from Social Security)
  2. The insurance company notifies CMS (Centers for Medicare & Medicaid Services)
  3. CMS adjusts your Social Security deduction downward by the giveback amount
  4. You see a higher Social Security deposit each month

The adjustment typically takes 1-3 months to appear after enrollment.

If you pay Part B directly (not through Social Security), you will receive a check or credit instead.

How Much Can You Get Back?

Giveback amounts vary by plan and by ZIP code. In Florida for 2026, I have seen giveback amounts ranging from:

  • $10-$50/month -- common on many plans
  • $50-$100/month -- available in many Florida counties
  • $100-$174/month -- available in select high-competition markets
  • Up to $202.90/month -- full giveback, rare but available in some areas

The amount depends on your specific ZIP code and the plans available there. Urban areas with more competition (Miami-Dade, Broward, Palm Beach, Orange County) tend to have higher giveback amounts.

What Is the Catch?

There is no trick -- but there are trade-offs to understand:

1. These are still Medicare Advantage plans. You are trading Original Medicare's flexibility for a network-based plan. Make sure your doctors are in-network before enrolling.

2. Giveback amounts can change each year. A plan that gives back $150/month this year may reduce that to $75/month next year during Annual Enrollment Period. Always review your plan during AEP (October 15 - December 7).

3. The plan may have higher copays. A plan offering a large giveback may offset that generosity with higher copays for specialist visits, hospitalizations, or procedures. Look at the full picture, not just the giveback.

4. Drug coverage may be limited. Some giveback plans have more restrictive formularies. If you take expensive medications, verify your drugs are covered before enrolling.

Who Qualifies for the Giveback Benefit?

To receive the Part B giveback benefit, you must:

  • Be enrolled in Medicare Part A and Part B
  • Enroll in a Medicare Advantage plan that offers the giveback benefit in your ZIP code
  • Not be enrolled in Medicaid (dual-eligible beneficiaries have different rules)

There is no income test or application process beyond enrolling in the plan. If the plan offers a giveback and you enroll, you get it.

Is the Giveback Benefit Worth It?

It depends on your health and how you use Medicare.

The giveback is a great deal if:

  • You are relatively healthy and do not use a lot of medical services
  • Your doctors are in-network for the plan
  • Your prescriptions are on the plan's formulary
  • You want to reduce your monthly Medicare costs

The giveback may not be worth it if:

  • You have chronic conditions requiring frequent specialist visits or hospitalizations
  • Your preferred doctors are not in-network
  • You travel frequently and need nationwide coverage
  • You would prefer the predictability of Medigap

A $150/month giveback sounds great -- but if the plan has a $9,350 out-of-pocket maximum and you have a serious illness, you could end up paying far more than you saved.

How to Find Giveback Plans in Your ZIP Code

Not every ZIP code has giveback plans, and the amounts vary significantly by location. The best way to find what is available to you is to work with an independent broker who can search all plans in your area.

Florida Counties With Strong Giveback Options (2026)

Based on my experience placing clients across Florida, these counties tend to have competitive giveback plans:

  • Miami-Dade, Broward, Palm Beach -- High competition, often $100-$174/month available
  • Orange, Osceola, Seminole -- Strong options in the Orlando metro
  • Hillsborough, Pinellas -- Tampa Bay area has multiple giveback plans
  • Volusia, Flagler -- Daytona/Palm Coast area has solid options
  • Duval -- Jacksonville market is competitive

Rural counties typically have fewer options and lower giveback amounts, but plans are still available.
